What food inflammation testing actually measures

Food inflammation testing measures the immune system’s inflammatory response to specific food antigens. This is different from standard allergy testing in a way that really matters clinically.

Standard allergy tests identify IgE-mediated reactions: the immediate, dramatic responses that produce hives, swelling, or anaphylaxis within minutes. These are relatively easy to identify without testing because the reaction is obvious and fast.

Food inflammation testing targets delayed, non-IgE-mediated immune responses: primarily IgG antibody reactions and direct cellular inflammation. These unfold over 24 to 72 hours. The symptoms they produce are diffuse and easy to attribute to other causes: fatigue, joint pain, brain fog, bloating, skin changes, mood instability, weight resistance. A woman can eat the same inflammatory food every day for years, experience cumulative immune activation, and never connect her symptoms to what she’s eating.

The two inflammation pathways

IgG-mediated pathway: IgG antibodies bind to food antigens, forming immune complexes that circulate in the bloodstream and deposit in tissues. This triggers complement activation, mast cell degranulation, and sustained low-grade inflammatory response. Unlike IgE, IgG immune complexes produce systemic whole-body inflammation affecting multiple organ systems at once.

Direct cellular inflammation: Certain food components, including lectins, gluten-related peptides, and specific food additives, can directly activate intestinal epithelial cells and mucosal immune cells. This produces inflammatory cytokines like IL-1 beta and TNF-alpha without requiring antibody mediation. This is the mechanism by which intestinal permeability, what people call leaky gut, amplifies food-related inflammation.

Why this matters specifically after 40

Three things converge in midlife to make food-driven inflammation more impactful.

Intestinal permeability increases with age. The gut barrier becomes more permeable under chronic stress, dysbiosis, NSAID use, and hormonal changes. More food antigens reach the immune system and trigger more immune complex formation.

The cortisol-inflammation feedback loop. Chronic stress increases intestinal permeability by reducing tight junction proteins and dysregulates the immune system’s inflammatory response. A woman in sustained cortisol dominance is simultaneously more likely to be producing food-reactive antibodies and less capable of resolving the inflammatory response they trigger.

Declining estrogen changes immune regulation. Estrogen has significant immunomodulatory effects. As estrogen declines in perimenopause, inflammatory responses change, with increased reactivity to antigen exposure.

How it’s used clinically

A comprehensive food inflammation panel tests 132 to 200 food antigens and stratifies results by reaction level. The clinical protocol that follows has four stages:
